CONSENT, RELEASE, AUTHORIZATION AND REFUND
I am the parent or guardian of the registered child(ren). I consent to the participation of Music Together® classes operated by Enchanted Music Together, Inc., including but not limited to regular classes, demonstrations, trial classes, make-ups and any sing a longs. I understand that such programs have many different kinds of movements by their nature, and I assume all risk of injury arising from such activity, and accordingly I release, indemnify and agree to hold harmless Enchanted Music Together, Inc., Music Together LLC, and the owner and operator in which the applicable program is located, and their respective directors, officers, employees, sponsors, and staff from any claim, suit, demand, or action arising in connection with my child’s participation in programs operated by Enchanted Music Together, Inc. I further assume all responsibility for all transportation to and from such programs. I HAVE CAREFULLY READ THE ABOVE CONSENT, RELEASE AND AUTHORIZATION AND FULLY UNDERSTAND ITS CONTENTS. I UNDERSTAND THIS IS A RELEASE OF LIABILITY AND I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT VOLUNTARILY.
PHOTO AND VIDEO RELEASE FORM
My child and I are participants in the Music Together® program. I hereby give permission to Music Together and Enchanted Music Together, Inc. to take photographs and video recordings of my child, or me, or both of us together, and use them in printed, web image or video formats for advertising and educational purposes.
The images may be edited in your sole discretion, and I expressly release Music Together, your agents, employees, and assigns from all claims which I have or may have for invasion of privacy, defamation, or any other cause of action arising out of exhibition or distribution of any materials in which my child and/or I appear.
By signing this document, I represent and warrant that I am the parent or guardian of the child whose name appears on the registration, and that we shall both be bounded by this agreement.
Registraion Fee for new families:
Registraion fee for new familes is non-refundable. Course fee is refundable up to one week prior to the first class. A $75.00 cancellation fee is deducted after the the first class, no refunds thereafter.
Registration Fee is non-refundable. Course Fess is refundable until two week prior to the first class. An $85.00 cancellation fee is deducted on the first class if no materials were received. If materials were received and the download code is used there will be an additional $45.00 fee. No refunds after the first week of class. No refunds will be offered due to naps and scheduling conflicts, but a change in class time might be an option. Classes are not prorated for late starts. As a courtesy, unlimited make-up classes are available to be used within the collection that the classes are missed. Makeups cannot be rolled over to the next session/ collection.
